To obtain a registry number, the information required by section 8 of the Hazardous Materials Information Review Regulations and the Summary of Information must be submitted to the Hazardous Materials Information Review Commission.

Basic Information about the forms:

The Claim for Exemption Form and the Substantiating Information Form are printable and can be saved to a computer or on a disc.

Please note that you must have version 7.0.9 of Adobe Reader or higher in order to use these forms. You can download the latest versions of Adobe Reader from their website at: get.adobe.com/reader/

Suggestions: Before you begin, you may wish to create an HMIRC folder in the "My Documents" section of your computer or any other folder you choose and then save a "template" copy of the new PDF forms in that folder.

  • Start by entering all your tombstone information (i.e. company name, address etc.) on the blank forms, then go to the last page of the form and click Save a Copy to your Desktop.
  • Retrieve the Forms from your desktop and move them to the folder you've created. To help you locate the forms, you may wish to save them as "HMIRC Claim for Exemption Form template" and "Substantiating Information Form template". You then will be able to use these templates for any future claims you wish to submit.

In the Claim for Exemption Form:

  • All the tombstone information entered in PART I is automatically repeated in PART VII.
  • All the information you enter in the Product Identifiers (PI) section of PART III, will be repeated in PART IV and in PART VII.

*Please note that if you need to change the tombstone information on your form, you can only do so by changing it from PART I.

Submitting your Claim for Exemption Form:

Print two (2) copies of the completed Claim for Exemption Form, one for your files and one to remit to the Commission.

All confidential information supplied in Part VII and possibly Part IV is to be placed in a separate sealed envelope. Label this envelope with the claimant name and mark it CONFIDENTIAL BUSINESS INFORMATION.

Place this envelope within the main envelope along with remaining PARTS I to VI of the Claim for Exemption Form. Remember to also label the main envelope with claimant name and address.

Before you remit your application to the Commission, please ensure that the following documents are included in the main envelope:

  • the completed Claim for Exemption Form;
  • the applicable fee via cheque, credit card or money order and;
  • the Material Safety Data Sheets (MSDS) and label (if applicable) associated with your claim.

Remit to the Commission a signed original either in person, by courier or Registered Mail to the following address:
